Who is eligible for home care? People of all ages can benefit from care in their home. To be eligible for services, patients must meet three basic criteria:

Under a doctor’s plan of care
In need of a reasonable and necessary skilled care or therapy
Ability to leave home is restricted by illness or injury

Getting Started with Kenosha VNA. Our friendly intake staff can get home care services started quickly with the following information:

Patient name, address and phone
Patient’s payer source (Medicare, Medicaid, Insurance type)
Skilled need (nurse, PT, OT, ST) and expected frequency
Patient’s primary caregiver in home
Any pertinent diagnosis
